The Indiana Immunization Coalition will be at two Merrillville schools to provide immunizations, a great opportunity for families to meet the Merrillville Community School Corporation's vaccination deadline of Sept. 18.
The immunization clinics will be held
Sept. 7, from 10 a.m. to 3 p.m.
at Merrillville High School, 276 E. 68th Pl.;
& Merrillville Intermediate School, 1400 W. 61st Ave.
Anyone can get immunized at these clinics. Children do not have to be students at these schools to receive immunizations. Walk-ins are welcome, but online registration is preferred; without a minimum number of online registrants, the clinic may be canceled.
Register online today: https://patients.vaxcare.com/registration
and enter code IN65942.
In addition, download, print, and complete this screening questionnaire, and bring it to the clinic.
Vaccines that may be administered include DTaP/Tdap, Hepatitis A, Hepatitis B, HiB, Polio, Rotavirus, Influenza, MMR, Human Papilloma Virus, Meningitis and Varicella.
The coalition accepts all forms of insurance, and there is no cost for those without insurance, with the exception of the Shingles vaccine.
The Indiana Immunization Coalition will vaccinate with all Advisory Committee on Immunization Practices (ACIP) recommended vaccines, unless they are contacted for specific vaccine refusal 48 hours prior to the clinic via email to firstname.lastname@example.org or phone (317) 628-7116.
